All services and treatment must be paid in full at the time of service in order to take advantage of the 20% discount. It is non-transferrable or refundable. Unused benefits do NOT carry over into another 12-month period. Your plan covers treatment in our office and does not extend to specialists to whom you may be referred. Your plan cannot be used in conjunction with any other discounts or discount program. The plan cannot be used for treatment involving injury covered under Workman’s compensation or injuries being litigated. The cost of the program must be paid in full before benefits take effect. Periodontal Services will be discounted at 20%. Excludes whitening, medication, and products.
